Anthropic published a report identifying three overlapping product changes that led to quality complaints about Claude Code over six weeks. The issues included a downgrade in reasoning effort, a caching bug that progressively erased the model's own reasoning, and a verbosity limit on system prompts causing a 3% drop in quality. Importantly, the API and model weights were unaffected, and all issues were resolved by April 20.
